As defined by the Occupational Safety and Health Administration, an incident is any occurrence, condition, or situation occurring while performing work duties that resulted in or could have resulted in injuries, illnesses, damage to health, and fatalities.
It is also common to use the term "accident," which refers to a situation in which an event occurs that is unplanned and may (or may not) cause injury or property damage. Occasionally, the two terms are used interchangeably. Incidents are events that may cause injury or damage in the future but did not occur at the time. Similarly, "near misses" or "dangerous occurrences" refer to events that could have caused harm but did not.
Employers, supervisors, health and safety committee members, or anyone involved in an incident investigation team can use the information below as a general guide. Investigators should focus on determining the root cause of incidents so they can prevent them from repeating themselves. It is not about finding fault, but about finding facts that could lead to corrective actions. Find the root cause always.
